Before you begin
To receive ADHD medication management services, you must meet one of the following requirements:
Option 1: Complete Christa Cusimano’s ADHD Evaluation
- A full diagnostic evaluation with Christa Cusimano, Psy.D. at SRC Center of Attention
- Required if you’ve never been diagnosed or never received prescription treatment for ADHD

- Never received prescription treatment for ADHD
- Required for clients who already have a diagnosis or treatment history

Follow-up appointments are $250 per month and include 20-minute sessions focused on medication monitoring, ADHD coaching, and therapy adjustments.
Evaluation / Assessment
Essential ADHD Evaluation
Best for: Individuals seeking a professional ADHD diagnosis for medication evaluation, self-knowledge, or basic documentation needs.
Includes:
- Comprehensive clinical interview and structured ADHD diagnostic interview (virtual or in-person)
- ADHD-specific self-report and collateral questionnaires
Basic psychological symptom screening - A 1-page Diagnostic Summary appropriate for medication referrals

Best for: Individuals needing a broader psychological evaluation for workplace accommodations or a more detailed understanding of psychiatric concerns.
Includes everything in the Essential ADHD Evaluation, plus:
- Expanded psychiatric symptom assessments
Review of selected medical, educational, or psychiatric records - Diagnostic considerations for ADHD and additional psychiatric conditions
- A 2–3 page Summary of Findings report with expanded results and recommendations

Full Psychological & Cognitive Evaluation
Best for: Those seeking a complete psychological profile for school accommodations, licensing exam accommodations, or clarification of ADHD and learning disabilities.
Includes everything in the Extended Profile, plus:
- Comprehensive cognitive (IQ) testing
Academic achievement/learning disability assessment - Diagnosis considerations for ADHD, learning disorders, and psychiatric conditions
- An 8+ page Comprehensive Psychological Report with tailored recommendations

Initial Psychiatric Evaluation

Best for: Individuals with an existing ADHD diagnosis who’ve completed an initial psychiatric evaluation and need brief, ongoing medication check-ins. This 20-minute session covers symptom updates, medication response, and any needed adjustments.
